From Here2Home provide Support Coordination to NDIS participants. In addition, they also offer a higher level of support coordination for people whose circumstances are more complex. This is delivered by a Specialist with a high level of experience who can support you to address barriers and resolve points of crisis.

From Here2Home also specialise in Exploring Housing Options. They will be able to support you to find the right home for you. This may involve returning to your existing home with In Home Support, or seeking a new home with Supported Independent Living (24/7 support). From Here2Home are able to assist with this transition to make it less confusing and ensure you receive the support you require.
