CTS provides all levels of support coordination and recovery coaching. CTS is a professional service with qualified and experienced team members who specialise in working in partnership with participants and offer a person centred service.

Team members are experienced in working with people in a variety of settings including community, accommodation, criminal justice system, mental health system, out of home care, education, child and family services and health.

Coast Transitional Support assist participants to explore and link into services that include informal supports, funded supports, mainstream supports and the community. CTS provide crisis planning, prevention, mitigation and action.
