Being well prepared for your plan reassessment means you’re more likely to get the supports you need for the duration of your plan.
We often hear from Leap in! Members that they feel nervous about their NDIS plan meetings. Being prepared and knowing what to expect is a great way to overcome those nerves.
The NDIS is updating some processes over time, so it’s important to prepare early and bring clear, current information about your support needs, goals and circumstances.
Here, we share how reassessments work and what you can do in advance to feel more confident going into your meeting.
What is a plan reassessment?
A plan reassessment is the process of reviewing your NDIS Plan and seeing whether any changes need to be made. The NDIS also calls this a “check-in”.
Every NDIS Plan includes a reassessment date. Before that date, the NDIS will be in touch to begin working with you on your next plan.
Depending on your circumstances, the NDIS may:
- Create a new plan
- Vary your current plan
- Continue your current plan.
Around two to three months before your reassessment date, your NDIS contact will get in touch to arrange a check-in. Your NDIS contact is the person responsible for working with you on your plan. Depending on your situation, this may be an NDIS planner, a Local Area Coordinator (LAC) or another contact such as an Early Childhood Partner.
The check-in is a conversation about how your current plan has been going and whether your supports still reflect your needs. It’s also your opportunity to ask for changes to your next plan.
A plan reassessment can take place in person, over the phone or via video call. Be sure to let your LAC or planner know if you require any assistive aids, an interpreter or will be taking a support person.
Start preparing early.
Give yourself plenty of time before your check-in to think about how your plan has been going and what you want from your next one.
The more clearly you can answer these questions, the easier it may be to explain the supports you need.
Some useful questions to think about include:
- How have you been tracking against your goals?
- Have your goals changed or do you have new ones? For example, are you finishing school, starting work or study?
- Are your current supports working well for you?
- Has anything not worked well in this plan?
- Has your situation changed since your last plan? Have you moved out of home? Have your informal supports changed? Have you received an additional diagnosis, or have your disability-related needs increased?
- Are you expecting any changes in the months ahead?
- Are you using services in your community alongside your NDIS supports?

Information and documents needed for your NDIS plan reassessment.
Being over-prepared is much better than being under-prepared. Use the guide below for helpful information to take to your meeting – you may not need all of these or have others you’d like to include as well.
1. Leap in! Plan Ready Summary.
- View, print or share this summary that you can find in the menu of the My Profile section in the app. Include any photos, information and status updates you or your crew have tracked for your goals during the plan
- A phone or tablet with the Leap in! app, so you can reference any detail you need for amounts spent, allocated or unspent.
2. Documents, assessments or reports.
- Recent reports from occupational therapists, speech pathologists or other allied health professionals, particularly if your support needs have changed
- Statements and reports from service providers showing how supports are helping you achieve your goals
- Recommendations from providers about supports or services you may need in future
- A copy of your most recent statement from Leap in! (or other plan manager), which shows how your budget has been spent
- Home modification or incontinence assessments and management plans.
The Leap in! app can help you organise these documents in one place. Simply upload documents to the Documents section of the app for easy access during your meeting.
3. Your community and mainstream supports.
You may also want to include:
- A current support plan from your service providers listing your supports
- A list of informal supports from family members, friends or the community.
These supports can also be added to the Crew section of the Leap in! app.
Some NDIS budget arrangements are changing as reforms roll out. From 1 October 2026, the NDIS will reduce Social, Civic and Community Participation supports and Capacity Building daily activities when plans are reassessed or renewed. Supports for critical care and core daily living needs are not expected to change. Read more about the NDIS reforms.
4. Quotes for new supports.
You may wish to bring quotes from providers for supports or equipment you would like included in your new NDIS Plan.
While this is not compulsory, it can help support conversations during your meeting.
Funding periods for all new and reassessed plans.
If your plan has been rolled over, continued or you have not had a plan reassessment since 19 May 2025, your next new or reassessed plan will include funding periods.
Funding periods mean the NDIS releases your budget in smaller portions across the length of your plan, rather than making the full amount available upfront.
When one funding period ends, the next portion of your funding becomes available.
NDIS plan meeting tips.
1. Take a support person.
If you feel more confident and able to clearly communicate your needs, goals and personal details with someone you know and trust by your side, ask that person to go with you.
Spend some time talking together before the meeting so they understand the process and your goals. That way they can help you communicate all the important pieces of information if required.
2. Ask questions.
Your reassessment meeting is an opportunity to ask questions about your NDIS Plan and supports.
Being prepared can help you feel more relaxed and confident during the discussion.
3. Be prepared to talk about your current plan.
Think about what has worked well and what hasn’t worked well in your current plan.
You may also want to think about how your supports have helped you work towards your goals and where you may need additional support.
4. Be clear on the details.
Preparation is important. Before your meeting, think about:
- How your disability impacts your daily life
- Your current goals
- The supports you need to work towards those goals
- The supports you currently use.
5. Keep records and evidence.
Keep records of your supports, reports, invoices and progress towards your goals.
Having clear records can make it easier to provide information and evidence if the NDIS asks for it during your reassessment.
6. Know how you want to manage your funding.
Research your options before your meeting so you feel confident deciding how you want to manage your funding.
You may choose to:
- Use a plan manager like Leap in!
- Self-manage
- Have your funding NDIA managed
- Use a combination of these options.
What happens if my needs change between reassessments?
Let the NDIS know as soon as your circumstances change or are likely to change.
This may include changes to:
- Your address
- Your disability support needs
- Your employment
- Support provided by family or friends.
These changes may make you eligible for an unscheduled assessment. In some situations, your existing supports and budgets may be used differently to meet changing needs without requiring a reassessment.
If the proposed legislative changes are passed, unscheduled plan reassessments will be limited. These will be harder to request and will only happen in limited circumstances. Your plan is unlikely to change outside your regular review cycle, unless there is a significant change in your support needs
