It’s common to feel nervous as the end of your NDIS Plan approaches, especially if you’re unsure what to expect.
Understanding how check-ins and plan reassessments work can help you feel more confident and ensure the NDIS up-to-date information about your support needs, goals and circumstances.
Not everyone will receive a different plan when their current plan ends. Here, we explain what to expect and how you can prepare.

What is a plan reassessment?
A plan reassessment is when the NDIS replaces your current plan with a new plan containing different supports.
Every NDIS Plan includes a reassessment date. Your my NDIS contact will generally arrange a check-in two to three months before this date to discuss how your plan is going and whether anything has changed.
Depending on your circumstances, the NDIS may:
- Vary your current plan if only small changes are required
- Continue your current plan for another 12 months (before 1 February 2027)
- Create a new plan with different supports.
Important update.
Changes to the NDIS will affect what happens when a plan reaches its reassessment date.
- Before February 2027: The NDIS may continue your existing plan for 12 months. Your Core and Capacity Building funding will generally remain the same, with indexation applied, and any unspent funding will remain available in the continued plan.
- From 1 February 2027: A renewed plan will be created when a plan reaches its reassessment date. Unused funding from the old plan will no longer automatically carry over.
What to expect from a check-in.
About two to three months before your plan reassessment date, your my NDIS contact will check in to discuss how your plan is going and whether your supports still meet your needs. Depending on your circumstances, your contact may be an NDIS planner, Local Area Coordinator (LAC) or Early Childhood Partner. You might be asked:
- How you’re tracking towards your goals
- Whether you have new goals or want to change your current goals
- How you’re using services in your community and the NDIS supports in your plan
- How your NDIS supports are helping you work towards your goals
- Whether your NDIS supports still meet your disability-related needs
- If your circumstances have changed or are likely to change soon.
If only small changes are needed, you may not require a plan meeting. If the NDIS varies or continues your plan (before 1 February 2027), the updated plan will be automatically uploaded to the participant portal and my NDIS app.
What happens if you need a plan reassessment.
If your situation has significantly changed and your current plan doesn’t meet your needs, your my NDIS contact will start the plan review and reassessment process (this was previously called an NDIS plan review). They will explain:
- What information and evidence you’ll need
- How to gather and provide the information
- What will happen during the reassessment process
- How to prepare for your plan meeting.
How to gather information and evidence for a plan reassessment.
Start preparing early so you have time to collect current information and address any gaps. Your my NDIS contact can advise which documents are relevant to your circumstances, so you may not need everything listed below.
1. Prepare your Leap in! Plan Ready Summary.
You can view, print or share your Plan Ready Summary from the My Profile menu in the Leap in! app. Before your meeting, update it with any photos, notes or progress your crew has recorded against your goals.
Take a phone or tablet with the Leap in! app to your meeting so you can check how much funding has been spent, allocated or remains available.
2. Request current reports and assessments.
Contact your providers early and ask for any reports or assessments you need. Let them know when your plan meeting is and give them enough time to prepare their information.
Depending on your circumstances, this may include:
- Recent reports from occupational therapists, speech pathologists or other allied health professionals
- Provider reports explaining how your supports are helping you work towards your goals
- Recommendations from providers about supports or services you may need in your next plan
- Your latest statement from Leap in! (or other plan manager), which shows how your budget has been spent
- Home modification or incontinence assessments and management plans.
Upload your documents to the Documents section of the Leap in! app so they are organised and easy to access during your meeting.
3. Record your community and mainstream supports.
Ask your providers for an up-to-date support plan and make a list of the help you receive from family, friends and community services. You can add these people and supports to the Crew section of the Leap in! app.
From 1 October 2026, the NDIS will reduce Social, Civic and Community Participation supports and Capacity Building daily activities when plans are reassessed or renewed. Supports for critical care and core daily living needs are not expected to change. Read more about the NDIS reforms.
4. Obtain quotes for new supports.
If you want new supports or equipment considered for your next plan, ask relevant providers to prepare quotes. Check with your my NDIS contact whether any additional information or assessments are required to support your request.
NDIS plan meeting tips.
- Take a support person: Choose someone you know and trust. Spend some time talking together before the meeting so they understand your goals and how you would like them to support you during the meeting.
- Review your current plan: Be prepared to talk about what has worked well, what hasn’t and whether your needs or circumstances have changed
- Organise your information: Group your reports, assessments and quotes by budget category so they are easy to find and discuss
- Decide how you want to manage your funding: You can use a plan manager such as Leap in!, self-manage, choose NDIA management or use a combination of these options
- Prepare for funding periods: All new and reassessed plans now include funding periods, with portions of your budget made available at set intervals throughout your plan. Consider whether you have any specific requests you would like to discuss.
