What happens at the end of my NDIS Plan? A guide to check-ins and plan reassessments.

It’s common to feel nervous as the end of your NDIS Plan approaches, especially if you’re unsure what to expect. Understanding how check-ins and plan reassessments work can help you feel more confident and ensure the NDIS up-to-date information about your support needs, goals and circumstances. Not everyone will receive a different plan when their current plan ends. Here, we explain what to expect and how you can prepare. What is a plan reassessment? A plan reassessment is when the NDIS replaces your current plan with a new plan containing different supports. Every NDIS Plan includes a reassessment date. Your my NDIS contact will generally arrange a check-in two to three months before this date to discuss how your plan is going and whether anything has changed.
